Our three Buffalo practices have received the National Committee for Quality Assurance’s (NCQA) Patient-Centered Medical Home Recognition, achieving Level 3 status; the highest designation offered.

NCQAPatient centered means that you come first. Medical home means that we are here to help you meet your health care needs, from coordinating your preventive and acute care, to helping you manage your chronic conditions.

Your team: Your doctor leads a team of health care professionals to coordinate and provide your total health care - and you are at the center of the team. We work in partnership with you to help you meet your health care goals and make informed decisions about your care.
