BUFFALO AREA PATIENTS
After much consideration, we have made the difficult decision to close our three Buffalo area health centers: Amherst Health Center, Hamburg Health Center, and William E. Mosher Health Center on December 31, 2017. All medical practices as well as behavioral health and specialty services (dental, imaging, etc.) at these locations will close as of that date. The pharmacies at these sites have been sold to Rite Aid and will close by October 25, 2017.

We are sorry to be leaving and understand that this news may be concerning to you. Please know that making sure you continue to have access to quality medical care is our number one priority. 


TO STAY WITH YOUR LIFETIME HEALTH DOCTOR

Many of the Lifetime Health providers are hoping to remain in the Buffalo area to continue your care. As physicians decide on their new office location*, current LIfetime Health patients will receive a letter with the new practice name, address, and phone number of their physician so that they may follow him or her. 
*For any new locations Lifetime Health learns of prior to December 31, 2017.


PHARMACY

Amherst Health Center Pharmacy
Amherst Health Center Pharmacy will close at the end of the business day on Tuesday, October 24, 2017, and existing prescriptions will be transferred to Rite Aid Pharmacy at 3249 Sheridan Drive in Amherst. If you have a prescription with the Amherst Pharmacy, beginning Wednesday, October 25, you can fill new or refill medications at this Rite Aid location. 

Hamburg Health Center Pharmacy
Hamburg Health Center Pharmacy will close at the end of the business day on Monday, October 23, 2017, and existing prescriptions will be transferred to Rite Aid Pharmacy at 140 Pine Street in Hamburg. If you have a prescription with the Hamburg Pharmacy, beginning Tuesday, October 24, you can fill new or refill medications at this Rite Aid location.

Mosher Health Center Pharmacy
Mosher Health Center Pharmacy will close at the end of the business day on Wednesday, October 25, 2017, and existing prescriptions will be transferred to Rite Aid Pharmacy at 350 Niagara Street in Buffalo. If you have a prescription with the Mosher Pharmacy, beginning Thursday, October 26, you can fill new or refill medications at this Rite Aid location.


MEDICAL RECORDS

Whether you follow your Lifetime Health doctor or choose a new provider, HIPAA privacy regulations require that you must have your medical records transferred. Please contact your health insurer to be sure that the doctor you select participates with your health plan. We are happy to assist you with the transfer of your medical records.

Call the Medical Records helpline at 716-885-2833 extension 56005. Representatives will be available Monday through Friday, from 8:00am to 5:00pm to take your calls. This phone line will remain operational until March 30, 2018.

As a courtesy, we will copy and forward the most recent two years of the record(s) to the provider of your choice free of charge. Record information prior to that time is available at a charge of 75 cents per page for copying costs, as permitted by New York State law.
