Medical Records

Whether you are following your Lifetime Health doctor or choosing a new provider, HIPAA privacy regulations require that you must have your medical records transferred. We are happy to assist you with this. Please fill out a Release of Information Authorization Form if you haven’t already and return it to us by fax or mail.

We also have a helpline to assist you with the transfer of your medical records at 716-885-2833 extension 56005. Representatives will be available to take your calls Monday through Friday, from 8:00 am to 5:00 pm through March 30, 2018.


The Lifetime Health Business Office is available to assist you with your account questions, concerns and payment options through March 31, 2018. For your convenience:

  • We offer online bill pay here.
  • We can help you set up a budget plan to make bi-weekly or monthly payments through March 31, 2018.
  • You may qualify for a discount of up to 25% (or more depending on the amount outstanding), if you resolve your balance by March 31, 2018.

If you have x-rays that have been taken by Lifetime Health Medical Group and would like to have them transferred to a new practice, you need to complete the Release of Information Authorization Form, and return it to our Medical Records department. X-rays will then be released to you directly or to the provider of your choice.

Patient Portal

After December 29, secure messaging with your doctor on MyLifetimeHealth and log in support will no longer be available. From December 30 on, you will be able to access your historical medical information, including messages that were sent or received up until December 29.
