Important Information for You

As you know from recent communications, Lifetime Health Medical Group will be transitioning ownership of our Rochester primary care practices and services to UR Medicine and Rochester Regional Health on January 1, 2018. We expect this change will enhance and improve your access to care, as both of the systems offer extensive primary and specialty care resources.

The Greece, Perinton and Wilson practices will transition to Rochester Regional Health and all will remain in their current locations, including Pharmacy and Urgent Care services.

The Artemis, Westfall and Folsom practices will transition to UR Medicine. If you are a patient of Artemis or Westfall, you can continue your care at the same building with your provider. For Folsom Primary Care patients:

  • Dr. Rosario Soriano and Physician Assistant Jacqueline Malay will be practicing at UR Medicine Primary Care Medical Associates of Henrietta, 300 Red Creek Drive, Suite 200, and her new phone number after January 1, 2018 will be 585-487-2221.
  • Dr. Brenda Davis will be practicing at UR Medicine Primary Care Panorama Internal Medicine at 2212 Penfield Road in Penfield, and her new phone number after January 1, 2018 will be 585-598-8555.

If you are currently a patient of the Lifetime Health Eye Services practice, you should have received more information from UR Medicine about new nearby locations for you to continue your care.

For patients of Urgent Care by Lifetime Health

Patients can continue to use Urgent Care at the Greece and Wilson locations. For patients who previously used Urgent Care at the Folsom location, you can now go to:

TO CONTINUE TO SEE YOUR PRIMARY CARE PROVIDER

If you want to continue to see your current provider after January 1, you will not need to do anything else to continue your care. Simply call the practice to schedule appointments as you have. We recommend that you contact your health insurer to confirm they participate with the health system taking ownership of the practice.

If you prefer to choose a new physician, please see the list of area health care practices that are currently accepting new patients (this list was also included with your notification letter). Please remember to verify the new provider’s participation with your health insurer.

We can transfer your medical records to your new doctor with your signed consent. Please fill out a Release of Information Authorization Form if you haven’t already and return it to us by fax or mail.

MYLIFETIMEHEALTH PATIENT PORTAL

Through December 29, 2017, you will be able to access the portal and do everything as normal if you have an account. After December 29, 2017:

  • For Greece, Perinton, and Wilson Patients: You can continue to log in to your portal account as you have been, however after December 29, you will not be able to send or receive any new information in the portal. You will be able to see the medical history, medication list, and any messages to and from your provider that were in your account prior to December 29.
  • For Artemis/Westfall Patients: You will be able to log in, send and receive secure messages from your provider, and access medical history, older messages, and all other aspects of the portal you are used to. You will see a different look to the portal for UR Medicine, but all aspects will remain the same, including how you log in.
  • For Folsom Patients: You can continue to log in to your portal account as you have been, however after December 29, you will not be able to send or receive any new information in the portal. You will be able to see the medical history, medication list, and prior messages to and from your provider that were in your account prior to December 29. The portal will also have a different look for UR Medicine.

BILLING

The Lifetime Health Business Office is available to assist you with your account questions, concerns and payment options through March 31, 2018. For your convenience:

  • We offer online bill pay here. You can also submit payments to us at Billing Department, PO Box 67340, Rochester, NY 14617.
  • We can help you set up a budget plan to make bi-weekly or monthly payments through March 31, 2018.
  • You may qualify for a discount of up to 25% (or more depending on the amount outstanding), if you resolve your balance by March 31, 2018.
