Migraine Materials Request Form

Lifetime Health Medical Group’s Acute Migraine Treatment Center was specially designed to treat the symptoms and address the needs of acute migraine patients aged 16 and older. For those patients whose standard medication therapies are unsuccessful in providing relief, the center provides an alternative to the wait and expense of the emergency room. The center is located within our Urgent Care by Lifetime Health facility at the Wilson Center at 800 Carter Street in Rochester.

We are happy to provide your office, free of charge, with patient information cards about the Migraine Treatment Center.

To request a supply of cards, simply complete the form below. Please order in multiples of 25, and note that orders require 7-10 business days for processing and delivery.
