Online Health Management

This section offers our patients many tools to manage their medical care. Here you will be able to:

  • Click on the MyLifetimeHealth* bar to the left to communicate with your doctor online. Request appointments, get lab results and send and receive secure messages.
  • Search our health library for information in the Healthwise Knowledgebase. You can learn about more than 6,500 health conditions and treatments, tests and medications.
  • Obtain important forms, including forms for Advance Care Planning and a risk factor and health history questionnaire.
  • Link to other health-related Web sites under our Wellness Links section. The links are alphabetized and divided out by topic.
  • Pay your medical bill online, or ask our billing department a question.

*Hamburg and Perinton Health Center patients are currently eligible for MyLifetimeHealth. If you are patient of another Lifetime Health practice, please look for more information soon. Current RelayHealth users at other practices may continue to use this service.
