Insurance Accepted

Lifetime Health Medical Group participates with nearly all major commercial insurance companies and submits claims to the insurance carrier for you. You are responsible for paying your copayments and/or deductible, or for any service that is not covered under your contract.

Because there are a wide variety of insurance options, please contact your insurance company directly to make sure that your Lifetime Health Medical Group provider participates with your insurance plan before seeking treatment.

If Lifetime Health does not participate with a commercial insurance company, our business office will submit the initial claim for payment to the insurance company as a courtesy. If the insurance company sends its payment directly to you, please forward the payment to Lifetime Health Medical Group. The amount of the payment is based on the terms of your insurance plan. If Lifetime Health does not participate with an insurance company, Lifetime Health is not responsible for collecting payment from the insurance company or negotiating a settlement for a disputed claim with the company.

We do our best to verify your coverage and policy limitations. However, it is your responsibility to let us know about any changes to your coverage as soon as possible. If you have questions about your insurance coverage, please contact your insurance company.
