From simple matters, such as getting an appointment, to more difficult issues, like solving major service concerns, Patient Service Representatives are available to you Monday - Friday 8:00 AM - 4:30 PM, closed for lunch from 11:30 AM - NOON. Call the main number for your health center and hold on the line.

To change your primary care physician, call your health insurer's Member Services department. Please note we are not accepting new patients at the Hamburg Health Center at this time, due to full provider panels.
