You will need to provide your name, address, date of birth, an active email address and the name of your Health Plan. Important Note for Patients Using RelayHealth: We have recently received reports from patients in the Buffalo region who have experienced delays with the RelayHealth system, and are working to resolve the associated technical issues. If you experience any unusual delay contacting your provider through RelayHealth, please call your health center directly. With RelayHealth and an online connection, you can access numerous features from the comfort of your home, including:

What is a webVisit®?A webVisit helps you seek care from your doctor when you have a non-urgent medical need. You will be asked a series of questions about your specific concern. Your answers will help your doctor provide you with the best possible care and may save you a trip to the office. Will I Be Charged For Using This Service?There is no charge for routine communications, such as appointment and lab requests. WebVisits require a copayment or an out-of-pocket fee. Is My Privacy Protected?Unlike e-mail, a RelayHealth message uses secure encryption technology. Only you, your doctor, and your doctor’s authorized staff will have access. |
