All Lifetime Health Medical Group practices are nationally recognized as Level 3 Patient Centered Medical Homes*; the highest designation offered by the National Committee for Quality Assurance - and we are proud to be your medical home. We provide primary care for nearly 80,000 patients in Buffalo and Rochester. Urgent Care is available at seven of our locations in Buffalo and Rochester. A variety of services are offered at most sites, including pharmacy, laboratory, radiology, internal medicine, pediatrics, family practice and specialty care. All major insurances are accepted.
